    Endorsement Forms for State Senate

    INSTRUCTIONS FOR EMAILING FORMS:  Complete a form below, and  "SAVE AS."  When naming the form, please use your county FB abbreviation and the office name (for example, in the 20th LD Senate race, Thurston County would SAVE AS "TCFB_Senate20"). 

    Then email the saved form.  Otherwise, the formatting is lost if you try to email directly from your internet browser. 

    Please note that the gray text boxes should be filled in with the requested information, such as County Farm Bureau name, etc.

    Please fax or mail the signed copy for our records.
